For the Five Hundred Worlds
According to some lore I've come across (and which might be retconned by now but was official at some point), the helmet that's most commonly associated with cobbled-together "Mk.V" armor suits was salvaged from Terminator prototypes. There hasn't been any official visual representation for the sort of Terminator armor that such a helmet could've come from, so I put together an early type of Indomitus pattern battleplate and gave it Mark Five style helmets. Pretty satisfied with the result, personally.
